Beginning of School Year Forms
Required Forms
- Emergency Care Information - Form required for all students. Completed through weCare via SIS ParentVUE account.
- Health Form - Form required for all students. Submit in SIS ParentVUE or e-mail completed Health Information Form to Kimberly Boggan at [email protected].
- Computer Opt-out form - Only for students who are not taking home a computer.
- Parent Consent for Digital Resources - This consent allows your child to use FCPS-approved digital tools that require parental consent. It must be renewed every school year. (NOTE: will be updated for 2024-25 in SIS ParentVUE).
- Student Rights and Responsibilities (SR&R) Parent Signature Sheet - For information and to view the SR&R document for the current school year, visit the FCPS SR&R page. This form will be collected during our Learning Seminar classes on or before October 28th.

Free and Reduced Lunch Application
Consent to share information for benefits for other programs: If you have children eligible for the federal Free and Reduced-Price Meals program, they may also be eligible to participate in other Fairfax County Public Schools (FCPS) fee-based programs without paying a fee or by paying a reduced fee. You are also eligible to receive information about scholarships, classes, and services that are provided by agencies and organizations other than FCPS.
If you want your children to participate in FCPS fee-based programs on a free or reduced fee basis and receive information about other benefits for your children, you must provide written agreement allowing FCPS staff to share information about your children's meal eligibility status.
